Before your child's physical appointment, you will need to fill out the
 2023-24 Annual Pre-participation Physical form.  

  1. FILL OUT Electronically, with Adobe Reader

  • go to above button, save the pdf to your computer and/or open the pdf with ADOBE READER (free to download if you don't have it). Next, fill out it and save to computer again.

  • Then, call our office (520.298.3383) and request that we send you an email with a link to our secure/encrypted message center (neocertified).  You can then attach the form to that secure message center.

2. ONLINE /PRINT:  Fill out the form online (see above instructions), print, and bring printed form to your appointment. 

3.  PRINT FORM:  Print form at home, fill out by hand (with pen), and bring to appointment.
